An investigation is under way after the death of an inmate at a jail in Greater Manchester. Dean Clementson, who was 33, died at HMP Forest Bank in Salford on 1 April. The Prisons and Probation Ombudsman confirmed the death on its website and said it had launched an independent investigation into the circumstances.
